Change in Utility Service

If you need to increase or decrease your service or would like to change from non-metered water/sewer to metered then fill out this form and submit it to City Hall.

Water/Sewer Meter Installation Form

To request installation of a water meter to take advantage of the lower rate you need to complete the Meter Installation Form and submit it to City Hall for processing.

Request for Change in Billing Information

If you need to change your billing information complete this form and submit it into City Hall.

Dumpster Rental Application and Deposit Form

Use this form to rent a dumpster on a regular basis or for short term projects. Complete and submit to City Hall.
